Das Gewichtsstigma und insbesondere das internalisierte Gewichtsstigma sind bei Kindern und Jugendlichen mit negativen Folgen für die physische und psychische Gesundheit assoziiert. Da die Befundlage in diesem Altersbereich jedoch noch unzureichend ist, war es das Ziel der Dissertation, begünstigende Faktoren und Folgen von gewichtsbezogener Stigmatisierung und internalisiertem Gewichtsstigma bei Kindern und Jugendlichen zu untersuchen. Die Analysen basierten auf zwei großen Stichproben, die im Rahmen der prospektiven PIER-Studie an Schulen rekrutiert wurden. Die erste Publikation bezieht sich auf eine Stichprobe mit Kindern und Jugendlichen im Alter zwischen 9 und 19 Jahren (49.2 % weiblich) und untersuchte den prospektiven bidirektionalen Zusammenhang zwischen erlebter Gewichtsstigmatisierung und Gewichtsstatus anhand eines latenten Strukturgleichungsmodells über drei Messzeitpunkte hinweg. Die anderen beiden Publikationen beziehen sich auf eine Stichprobe mit Kindern und Jugendlichen im Alter zwischen 6 und 11 Jahren (51.1 % weiblich). Die zweite Publikation analysierte anhand einer hierarchischen Regression, welche intrapersonalen Risikofaktoren das internalisierte Gewichtsstigma prospektiv prädizieren. Die dritte Publikation untersuchte anhand von ROC-Kurven, ab welchem Ausmaß das internalisierte Gewichtsstigma mit einem erhöhten Risiko für psychosoziale Auffälligkeit und gestörtes Essverhalten einhergeht. Im Rahmen der ersten Publikation zeigte sich, dass ein höherer Gewichtsstatus mit einer höheren späteren Gewichtsstigmatisierung einhergeht und umgekehrt die Gewichtsstigmatisierung auch den späteren Gewichtsstatus prädiziert. Die zweite Publikation identifizierte Gewichtsstatus, gewichtsbezogene Hänseleien, depressive Symptome, Körperunzufriedenheit, Relevanz der eigenen Figur sowie das weibliche Geschlecht und einen niedrigeren Bildungsabschluss der Eltern als Prädiktoren des internalisierten Gewichtsstigmas. Die dritte Publikation verdeutlichte, dass das internalisierte Gewichtsstigma bereits ab einem geringen Ausmaß mit einem erhöhten Risiko für gestörtes Essverhalten einhergeht und mit weiteren psychosozialen Problemen assoziiert ist. Insgesamt zeigte sich, dass sowohl das erlebte als auch das internalisierte Gewichtsstigma bei Kindern und Jugendlichen über alle Gewichtsgruppen hinweg relevante Konstrukte sind, die im Entwicklungsverlauf ein komplexes Gefüge bilden. Es wurde deutlich, dass es essentiell ist, bidirektionale Wirkmechanismen einzubeziehen. Die vorliegende Dissertation liefert erste Ansatzpunkte für die Gestaltung von Präventions- und Interventionsmaßnahmen, um ungünstige Entwicklungsverläufe in Folge von Gewichtsstigmatisierung und internalisiertem Gewichtsstigma zu verhindern.
This study examines the access to healthcare for children and adolescents with three common chronic diseases (type-1 diabetes (T1D), obesity, or juvenile idiopathic arthritis (JIA)) within the 4th (Delta), 5th (Omicron), and beginning of the 6th (Omicron) wave (June 2021 until July 2022) of the COVID-19 pandemic in Germany in a cross-sectional study using three national patient registries. A paper-and-pencil questionnaire was given to parents of pediatric patients (<21 years) during the routine check-ups. The questionnaire contains self-constructed items assessing the frequency of healthcare appointments and cancellations, remote healthcare, and satisfaction with healthcare. In total, 905 parents participated in the T1D-sample, 175 in the obesity-sample, and 786 in the JIA-sample. In general, satisfaction with healthcare (scale: 0–10; 10 reflecting the highest satisfaction) was quite high (median values: T1D 10, JIA 10, obesity 8.5). The proportion of children and adolescents with canceled appointments was relatively small (T1D 14.1%, JIA 11.1%, obesity 20%), with a median of 1 missed appointment, respectively. Only a few parents (T1D 8.6%; obesity 13.1%; JIA 5%) reported obstacles regarding health services during the pandemic. To conclude, it seems that access to healthcare was largely preserved for children and adolescents with chronic health conditions during the COVID-19 pandemic in Germany.

The Eating Disorder Examination adapted for children (ChEDE) is the child version of the semi-structured gold standard eating disorder interview for adults. This study was a comprehensive test statistic evaluation of the German ChEDE in a large sample of children and adolescents with anorexia nervosa, binge eating disorder, loss of control eating, overweight and obesity, as well as non-eating-disordered and chronically ill control probands (n=352). Excellent inter-rater reliability, adequate internal consistency and satisfactory stability of ChEDE indicators were demonstrated. ChEDE indicators discriminated between diverse forms of eating and weight disturbances and normative eating and were significantly correlated with conceptually related measures. Factorial validity was not convincing; a brief eight-item scale showed the best fit. Item statistics were mostly acceptable. Overall, the ChEDE's German translation reliably and validly assesses psychopathology across the eating and weight disorder spectrum and facilitates international comparison of eating disorder research.
To provide further insight into stress generation patterns in boys and girls around puberty, this study investigated longitudinal reciprocal relations between depressive symptoms, dysfunctional attitudes, and stress generation, the process by which individuals contribute to the occurrence of stress in interpersonal contexts (e.g., problematic social interactions) or in noninterpersonal contexts (e.g., achievement problems). A community sample of N = 924 German children and early adolescents (51.8% male) completed depressive symptoms and dysfunctional attitudes measures at T1 and again 20 months later (T2). Stressful life events were reported at T2. Dysfunctional attitudes were unrelated to stress generation. Interpersonal, but not noninterpersonal, dependent stress partially mediated the relationship between initial and later depressive symptoms, with girls being more likely to generate interpersonal stress in response to depressive symptoms. Findings underscore the role of interpersonal stress generation in the early development of depressive symptomatology, and in the gender difference in depression prevalence emerging around puberty.
Background: The polymorphism in FTO gene (rs9939609) is known to be associated with higher BMI and body fat mass content. However, environmental factors can modify this effect. The purpose of the present study was to investigate an association between sport specialization and the rs9939609 SNP in FTO gene in the cohort of professional and amateur young athletes. Methods: A total number of 250 young individuals 8-18 years old living in Moscow or Moscow district participated in the study. Individuals were divided into 3 groups in accordance with their physical activity level: control group (n = 49), amateurs (n = 67) and professionals (n = 137). Amateur and professional athletes were subdivided into groups according to their sport specialization. Quantile regression was used as a regression model, where the dependent (outcome) variable was BMI, along with percentage of body fat mass, and the independent variables (predictors) were the rs9939609 SNP in FTO gene, physical activity (active versus inactive), sport specialization (aerobic, intermittent sports and martial arts), nationality, level of sport experience (in years), gender and percentage of free fat mass content. Results: The regression analysis revealed that physical activity and sport specialization had greater impact compared to FTO allele in the group of physically active individuals. Physical activity, in particular aerobic, had negative associations with body fat mass and BMI. The rs9939609 SNP in FTO gene is associated with physical activity and aerobic activity. The magnitude of association becomes significantly larger at the upper quantiles of the body fat mass distribution. Conclusion: Physical activity and sport specialization explained more variance in body composition of physically active young individuals compared to the FTO polymorphism. Effect of interaction of physical activity, in particular aerobic, with the FTO polymorphism on body composition of young athletes was found.
Die vorliegende Studie beschäftigte sich mit der Bedeutung der dysfunktionalen Einstellungen für die Entwicklung von depressiven Symptomen bei Kindern und Jugendlichen. Nach der kognitiven Theorie der Depression von Beck (1967, 1996) führen dysfunktionale Einstellungen in Interaktion mit Stress zu depressiven Symptomen. Es existieren allerdings nur wenige Studien, die die longitudinale Beziehung zwischen den dysfunktionalen Einstellungen und der Depressivität bei Kindern und Jugendlichen untersucht haben (Lakdawalla et al., 2007). Folglich kann noch nicht eindeutig geklärt werden, ob die dysfunktionalen Einstellungen Ursache, Begleiterscheinung oder Konsequenz der Depression sind. Als Datengrundlage diente eine Stichprobe von Kindern und Jugendlichen im Alter von 9 bis 20 Jahren, die im Rahmen der PIER-Studie zu dysfunktionalen Einstellungen, kritischen Lebensereignissen und depressiven Symptomen befragt wurden (Nt1t2 = 1.053; t1: 2011/2012, t2: 2013/2014). Querschnittliche Analysen zeigten hohe Assoziationen zwischen den dysfunktionalen Einstellungen, kritischen Lebensereignissen und depressiven Symptomen. Eine latente Moderationsanalyse wies nur bei den Jugendlichen auf signifikante Interaktion zwischen den dysfunktionalen Einstellungen und den kritischen Lebensereignissen in der Vorhersage depressiver Symptomatik hin. Im Längsschnitt zeigten latente Cross-Lagged-Panel-Analysen erwartungsgemäß, dass die dysfunktionalen Einstellungen und die Depressivität mit dem Alter immer stabilere Konstrukte darstellen, die sehr eng miteinander zusammenhängen. Eine diesem Modell hinzugefügte latente Moderationsanalyse konnte das kognitive Modell der Depression nach Beck weder bei Kindern noch bei Jugendlichen bestätigen. Die spätere depressive Symptomatik konnte lediglich durch Haupteffekte der früheren Ausprägung der Depressivität und der kritischen Lebensereignisse vorhergesagt werden. Diese Ergebnisse legen den Schluss nahe, dass es sich bei den dysfunktionalen Einstellungen eher um Begleiterscheinungen als um Risikofaktoren oder Konsequenzen der depressiven Symptomatik handelt.
