Background: The pathophysiology behind the subacute but persistent hypercoagulable state after traumatic brain injury (TBI) is poorly understood but contributes to morbidity induced by venous thromboembolism. Because platelets and their microvesicles have been hypothesized to play a role in post-traumatic hypercoagulability, administration of commonly used agents may ameliorate this coagulability. We hypothesized that utilization of aspirin, ketorolac, amitriptyline, unfractionated heparin, or enoxaparin would modulate the platelet aggregation response after TBI. Methods: Concussive TBI was induced by weight drop. Mice were then randomized to receive aspirin, ketorolac, amitriptyline, heparin, enoxaparin, or saline control at 2 and 8 h after TBI. Mice were sacrificed at 6 or 24 h after injury to determine coagulability by rotational thromboelastometry (ROTEM), platelet function testing with impedance aggregometry, and microvesicle enumeration. Platelet sphingolipid metabolites were analyzed by mass spectrometry. Results: ROTEM demonstrated increased platelet contribution to maximum clot firmness at 6 h after TBI in mice that received aspirin or amitriptyline, but this did not persist at 24 h. By contrast, adenosine diphosphate- and arachidonic acid-induced platelet aggregation at 6 h was significantly lower in mice receiving ketorolac, aspirin, and amitriptyline compared with mice receiving saline at 6 h after injury and only arachidonic acid-initiated platelet aggregation was decreased by aspirin at 24 h. There were no differences in microvesicle production at either time point. Platelet sphingosine-1-phosphate levels were decreased at 6 h in the group receiving amitriptyline and increased at 24 h along with platelet ceramide levels at 24 h in the amitriptyline group. Conclusion: After TBI, amitriptyline decreased platelet aggregability and increased contribution to clot in a manner similar to aspirin. The amitriptyline effects on platelet function and sphingolipid metabolites may represent a possible role of the acid sphingomyelinase in the hypercoagulability observed after injury. In addition, inhibition of platelet reactivity may be an underappreciated benefit of low molecular weight heparins, such as enoxaparin. (C) 2019 Elsevier Inc. All rights reserved.
With its exhortation “You shall also love the stranger (gēr), for you were strangers (gērîm) in the land of Egypt” (Deut 10:19), the book of Deuteronomy helps cultivate a healthy and appreciative sense of past hardship, current prosperity, progress, and relative privilege. In contemporary culture, where the term “privilege” has become an unfortunate source of contention, Deuteronomy might point a way for recognition of one’s relative privilege in regard to an Other as a basis for gratitude and responsibility. This essay argues that we have gained “privilege” after having been immigrants and strangers in a strange land. Privilege could become an empowering and challenging exercise of counting one’s blessings and considering how these could be used for the benefit of others, including strangers in our land.
Inspiriert durch die bis dato unveröffentlichten privaten Aufzeichnungen Ingeborg Bachmanns in dem 2017 erschienenen ersten Band Male Oscuro der neuen Ingeborg-Bachmann-Gesamtausgabe der Salzburger Bachmann Edition, untersucht die vorliegende Arbeit die Zusammenhänge zwischen dem Leben und Schreiben der österreichischen Autorin. Dabei werden die gedanklichen Parallelen zwischen den privaten Aufzeichnungen und Bachmanns Roman Malina als Idee für einen neuen Deutungsansatz genutzt. Hierbei steht jedoch bewusst statt einer typisch biografischen Lesart spezifisch die ästhetische Umsetzung der beobachteten Parallelen im Vordergrund, welche mithilfe von narratologischen, stilistischen und linguistischen Elementen auf ihre Stichhaltigkeit geprüft werden.
Die vorliegende Arbeit stellt daraufhin die These auf, dass Malina einen individuellen Verarbeitungsprozess von Traumata beschreibt, welcher gleichzeitig durch die im Text beschriebenen negativen Einflüsse der Gesellschaft auf diesen Prozess auch als eine umfassende Gesellschaftskritik aufgefasst werden kann. Diese Gesellschaftskritik ist jedoch nicht auf eine Faschismus- oder Patriarchatskritik reduziert, sondern lässt sich, gemäß der Ergebnisse dieser Arbeit, auf jegliche Formen des Machtmissbrauchs auf sämtlichen Ebenen der Gesellschaft beziehen. Die These stützt sich dabei auf ein close reading der im Roman beschriebenen traumatischen Erfahrungen, welche sich sowohl auf individuelle traumatische Erfahrungen (Inzest) als auch auf kollektive traumatische Erfahrungen (Holocaust) beziehen und dadurch der im Text veranschaulichten gesellschaftlichen Abwehr dieser Traumata eine besondere Wirkungskraft verleihen. Die narrativ-psychologisch motivierte Untersuchung des Romans knüpft dabei an den jüngeren Forschungsstand an und versucht sich an einer explizit werkimmanent argumentierenden Analyse, um so nahe liegende biografische Fehlschlüsse bewusst zu vermeiden. Nach einer Exposition des Themas und einem kurzen Überblick über die relevante Forschungsliteratur wird im Hauptteil die These handlungslogisch anhand spezifischer Textstellen untersucht und gleichzeitig mit stilistischen und narratologischen Detailanalysen unterstützt. Im Fazit wird die untersuchte These abschließend im Hinblick auf die eigene Erzählpoetik Bachmanns betrachtet und mit intertextuell anknüpfenden Themengebieten erweitert. Das Ende des Romans wird dabei mit Hilfe des narrativ-psychologischen Ansatzes positiv gedeutet, nämlich als die Lösung des im Verlauf des Erzähltextes bearbeiteten Problems: die Auflösung einer Trauma-bedingten Informationsblockade. Damit wird das bereits vielfach interpretierte Ende des Romans nicht als Tod der Ich-Figur gedeutet, sondern als Befreiung eben dieser aus dem Trauma-fördernden System der Romanwelt. Solchermaßen gelesen stellt Bachmann mit Malina einen - im Sinne der von ihr geforderten ‚neuen Literatur’ - durch und durch Erkenntnis anregenden, interdisziplinären Zusammenhang her, welcher die Beziehungen zwischen Trauma und Tabu, Krankheit und Gesellschaft, Opfer und Täter, Verarbeitung und Verdrängung kunstvoll zu beschreiben vermag. Der Roman bricht damit den Bann der Sprachlosigkeit rund um den zweiten Weltkrieg und veranschaulicht eindrucksvoll den individuellen Prozess eines traumatisierten weiblichen Ichs, das vom Vergessen über das Erinnern zum Ansprechen und anschließenden Überwinden seiner Traumata gelangt – und damit beispielhaft die Strukturen von Machtmissbrauch durchbricht.
