Unipolar depressive disorders in adolescence are common, lead to serious impairments and are often associated with comorbid disorders and a high risk for suicide. Thus, recognition and early treatment of depressive disorders are important. International and national treatment guidelines show that effective treatment approaches for prevention and acute therapy of depressive disorders are available. Based on current evidence and consensus-based guidelines, such as the Association of the Scientific Medical Societies in Germany (AWMF) S3 treatment guidelines for unipolar depressive disorders in children and adolescents, state of the art diagnostic procedures and treatment recommendations are proposed. Diagnostic procedures and differential diagnoses as well as differential indications and treatment planning are reported in detail. In the treatment section the focus is on cognitive behavioral therapy (CBT), which is currently the best evaluated psychological treatment form for depressive disorders in children and adolescents.

Es werden Befunde aus einer prospektiven Laengsschnittstudie praesentiert, in der 321 Probanden im Alter von 8, 13, 18 und 25 Jahren untersucht werden konnten; ihre Dunkelfelddelinquenz wurde mit 18 und 25 Jahren erfasst. Wir suchten nach Assoziationen zur Delinquenzentwicklung und erwarteten Unterschiede zwischen auf das Jugendalter beschraenkter gegenueber ins fruehe Erwachsenenalter fortgesetzter sowie spaet, d.h. nach dem Alter von 18 Jahren, beginnender Delinquenz. Wir fanden gemeinsame Risikofaktoren und fuer die drei Verlaufstypen spezifische Risikokonstellationen, die eher im Jugendalter als in der Kindheit identifiziert wurden. Widrige familiaere Bedingungen, Entwicklungsverzoegerungen und psychische Stoerungen scheinen mit Delinquenz als eher persistentem Verhalten assoziiert.

Use and misuse, age of first substance use, and prevalence of problematic use of legal (alcohol, nicotine) and illegal (cannabis etc.) drugs in adolescence were analyzed. The risk and protective factors for substance misuse were investigated. A representative sample of 246 14-15 year-olds were examined (20% of the population of a defined German region, cross-sectional design, self-rating inventory). Prevalence rates for problematic use were 19.9% for alcohol, 48% for nicotine, and 16.7% for illegal drugs. Mean age of first substance use was age 12 for legal drugs and age 14 for illegal drugs. 4,9% of the adolescents showed concurrent problematic use of nicotine, alcohol, and illegal drugs (male- female ratio: 5,2:1). Problematic substance use could be predicted best by a combination of risk factors (school failure, low level for availability of illegal drugs in the neighbourhood) and protective factors (participating a lot of sport, positive family climate). Suggestions for universal, selective, and indicated preventive interventions were derived.
